Abstract

Myeloproliferative Neoplasms Philadelphia Negative Chromosome (NPM Ph¯) are a group of chronic hematological diseases of course and heterogeneous evolution, treated with oral therapy and with great impact on patients’ quality of life.

International guidelines recommend that patients undergoing new, complex and long-term oral treatment drugs be followed up by multidisciplinary teams that promote patient and family literacy and empowerment for effective therapeutic regimen management and adherence.

With the translation and validation into the Portuguese language of a symptom load self-assessment tool recommended by International Guidelines (MPN-SAF TSS; MPN10), it allowed us to integrate nursing interventions in the traditional patient circuit with MPN in our hospital, hence creating a multidisciplinary meeting with medical, pharmacist and nurse presence, as a strategic measure to promote better management and adherence to the therapeutic regimen, since it allowed to approach the patients in
an innovative way, with a greater humanization in the provision of hospital care with the three classes professionals involved in the patient’s circuit. In This way, the Multidisciplinary Consultation allowed us to optimize data collection, analyze the knowledge and needs of the patient and enable him to develop care partnerships through an integrated, excellence-centered, patient-centered therapeutic plan.

We aim to publicize the importance of the role of nurses in the follow-up of patients with NPM Ph¯ in the context of a multidisciplinary consultation.
